Established multi site community health organization in North Central Massachusetts is seeking a Primary Care APP to join its growing outpatient practice. This is an expansion hire due to multiple openings and evolving community demand, with flexible scheduling, strong compensation, and meaningful incentive upside.
Job Description:
Compensation : $135000 - $180000 Job ID : 44586
Job Function:
* Outpatient primary care role serving primarily adult patients
* Accepting FNP, ANP, or Physician Assistants
* Flexible scheduling options:Full time or part time: 5x8s, 4x10s, 3x12s, or hybrid schedules. 1 evening per week required (4:00-8:00 PM)
* Patient volume expectations:2-3 patients per hour
* Adjusted expectations for providers seeing pediatric patients
* Incentive opportunities:Productivity bonuses for exceeding visit thresholds
* Retention bonus after 1 year
* Very light shared call:Approximately 1 weekend/month + 2 weekdays/month
* Structured onboarding with reduced patient load initially
* Practice locations include Fitchburg, Leominster, and Gardner
* Diverse patient population, including Brazilian, Portuguese, and Haitian Creole communities
* Translation services available and reimbursed
* Hybrid/telemedicine options may be explored
Job Requirements:
* NP (FNP or ANP) or PA
* Eligible for Massachusetts licensure (license not required to initiate process)
* Open to new graduates and experienced providers
* Strong clinical judgment and team oriented mindset
