
SUPPORT NURSE - LPN - THE FOLEY CENTER AT CHESTNUT RIDGE

We are currently seeking an experienced Support Nurse – LPN.
Responsibilities
Assist in ensuring the best patient care by supporting nursing personnel on a specified unit during assigned hours.
May be required to take call for staffing coverage.
Provide resident care in accordance with the Nursing Practice Act under the supervision of a Registered Nurse.
Delegate duties to non‑licensed nursing personnel and monitor performance under RN supervision; assist in preparing a work schedule as needed.
Assist in Quality Assurance data collection for analysis and corrective action implementation.
Support infection control for the unit.
Adhere to all cleanliness, infection control, and standard precaution regulations.
Manage records, orders, and assessments of new admissions; ensure initial nursing assessment is completed.
Complete assigned resident assessments as directed by the MDS Nurse or Director of Nursing.
Attend scheduled Nurse Department meetings.
Monitor and measure decubitus ulcers weekly, complete integument watch assessment forms accurately, and collaborate with other departments to implement unit and patient needs.
Report performance issues of employees to the RN for disciplinary considerations; assist in documentation and process as needed.
Assist with lab requests as ordered per facility policy; repost lab work, review results, and notify attending MD per established protocols.
Respond to emergencies on duty, perform CPR if needed, and assist on other units if required.
Notify attending MD of patient changes or illnesses; inform MD of emergencies and deaths; implement order changes as instructed.
Assist with general duties: rounds with doctors, checking MD orders, updating orders, verifying completeness of nursing summaries to ensure timeliness and accuracy.
May be asked to provide hall coverage as needed.
Coordinate the health care team, including rehab aide duties and assignments.
Other duties as assigned by the Director of Nursing in accordance with NC Board of Nursing Practice Acts.
Requirements
High school graduate.
Licensed Practical Nurse (LPN) with current, valid license in North Carolina; must submit proof of renewal.
Provide quality nursing care, follow doctors’ orders, and have a genuine interest in geriatric nursing.
CPR certified yearly; attend in‑service training and staff meetings as required.
Ability to make decisions concerning nursing and unit issues, recognizing that errors may have serious consequences.
Work well under pressure, problem‑solve, and perform various tasks.
Read, know, and follow personnel, departmental, and facility policies and procedures; adhere to local, state, and federal requirements.
Ability to maneuver medication carts, treatment carts, and other nursing equipment.
Background checks and a drug‑free workplace. EOE.
