Qualifications
Master’s degree from an accredited college or university in Social Work or Counseling.
LMSW or LPC from the State of Michigan. Highly qualified LLMSW may be considered.
Must meet qualifications of a Mental Health Professional (MHP) per Michigan PIHP/CMHSP Provider Qualification per Medicaid Services.
Duties and Responsibilities
Screen for co-occurring mental health and substance use disorders using integrated strength-based assessment interviews.
Assess acute needs and risk, and arrange appropriate staged interventions.
Conduct initial assessments to determine level of care based on contract criteria; analyze clinical histories for diagnosis and develop preliminary individualized treatment plans.
Obtain information from a variety of sources to form clinical history; record treatment plans and progress for case records.
Develop preliminary treatment plans for SA, MH, and COD that are individualized, person-centered, and reflect proper treatment matching; identify specific goals, objectives, and stage/phase-specific interventions.
Prioritize community resources and supports to assist individuals in meeting their needs.

Education & Experience
