Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24‑bed inpatient unit in Joliet. Non‑clinical staff are based out of our Joliet administrative offices.
Palliative Licensed Clinical Social Worker (LCSW)
The Palliative LCSW provides holistic support for patients with serious, life‑limiting illnesses and their families by addressing emotional, social, and practical needs. They provide counseling, advance care planning, and resource navigation, working alongside medical teams to improve quality of life.
Key Responsibilities
Provide counseling and assessments for patients and families facing grief, fear, or anxiety.
Guide families through advanced directives, living wills, and healthcare proxy decisions.
Lead family meetings to resolve conflicts and provide education on disease progression.
Link patients to community services, financial aid, and legal or insurance assistance.
Partner with medical staff to ensure care plans respect the patient’s personal goals.
Act as a liaison to protect patient dignity and ensure their final wishes are honored.
Participate in organizational initiatives to improve patient outcomes and service standards.
Stay current with healthcare regulations and best practices through ongoing training.
Monday through Friday schedule 9 AM – 5:30 PM.
Education/Experience
Current State of Illinois license: LCSW required.
Minimum of one year’s experience in health care; palliative or hospice experience preferred.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements; and be in good working order.
