Schedule: Sat 8a‑8p, Sun 8a‑8p, Mon 3p‑9p, Tue 3p‑9p
(We are also looking for PRN's As needed)

Group Homes
Job Purpose
To plan and carry out direct care of individuals with Developmental Disabilities Adult/Children.
Reports To
Group Home Manager
Key Responsibilities
Monitor and ensure the health and safety of all individuals living in the home.
Administer medications as prescribed by a medical professional.
Prepare all necessary meals (breakfast, lunch, dinner, snacks etc.).
Transport individuals to appointments, visits, and activities/community events.
Implement HAB objectives/goals as agreed upon by the ISP team.
When applicable implement behavior building plans.
Actively interact with individuals in the home throughout your shift.
Complete all required documentation before leaving shift.
Assist with preparing monthly activity calendar and weekly menus.
Ensure the home is clean and in good repair.
Other duties as assigned.
Qualifications
Clear criminal and central registry background check (Required)
At least 18 years of age
Valid driver’s license with clean driving record (If applicable)
Employee also agrees to maintain all required DDD certifications and a valid fingerprint card throughout the duration of employment with Sunny Field Residential Homes LLC.
Proficient with the English language: written and verbal
Education and Experience
High School Diploma, and minimum one year experience in the DDD or Behavioral Health Field.
Must apply for a Fingerprint Clearance Card within 7 days of hire.
Must have Article 9, Prevention and Support, CPR and First Aid certifications within 90 days of hire.
Immediate Hire if you possess all certifications
