Summary of Position
Under the direct supervision of the Site Coordinator, the Patient Representative is responsible for the accurate and timely recording of patient demographics, insurance information, patient charges, and payments. The role facilitates patient entry through Cabarrus Rowan Community Health Centers (CRCHC), including greeting clients, answering phone calls, and scheduling appointments.
Minimum Qualifications
Ability to read, write, and comprehend the English language.
Effective verbal and written communication skills.
Ability to sit and stand for prolonged periods.
Maintains confidentiality of records and communicates with personnel at all levels.
One year or less of clerical experience in an office setting.
Excellent organizational, problem‑solving, and critical‑thinking skills.
Strong basic computer skills.
Collaborative teamwork ability.
Preferred: bilingual in Spanish and English.
High school diploma or GED; Associate’s degree preferred.
Physical Requirements
Repetitive hand and finger movements – typing and/or writing.
Occasional standing, walking, stooping, kneeling, or crouching.
Reaching with hands and arms.
Talk and hear.
Key Responsibilities
Demonstrate high‑quality customer service to patients, staff, board members, providers, and vendors.
Verbalize knowledge of chain of command and direct supervisor information.
Report on time and manage schedule to organize patient flow.
Obtain payment information and create claims for reimbursement.
Understand required screening forms for UDS and other QI.
Maintain accurate documentation and registration updates in EMR.
Adhere to scheduling protocol for all appointment types.
Assist patients with appointments and reroute to triage as needed.
Explain the sliding scale program and process applications accurately.
Compose messages to providers/staff regarding patient communication.
Enter patient registration information into EMR.
Demonstrate knowledge of HIPAA, patient demographics, and benefits.
Follow and enforce safety policies while providing compassionate care.
Maintain patient confidentiality and secure protected health information.
Participate in mandatory in‑services, drills, staff meetings, and trainings.
Adhere strictly to infection control guidelines.
Work early, night clinic, and weekends as needed.
Travel to other CRCHC locations to cover as needed.
Provide translation services upon management request.
Perform other duties as assigned.
