Remote RN Care Coach for Chronic Care Management

A health management company is seeking a passionate remote RN Care Coach to provide education and support to Medicare patients. This part-time role requires excellent organization and communication skills to manage care for patients with chronic conditions. Ideal candidates will have an RN license, strong documentation skills, and a proven ability to work independently. Compensation is $15 per patient encounter paid monthly.
