Job Description
The Nurse Coordinator plays a vital role in the Lung Transplant Program(s), offering direct care to patients with complex medical conditions. They serve as the primary point of contact, coordinating pre‑transplant evaluations, managing post‑transplant follow‑up care, and providing patient education. This position requires collaboration with a diverse team—including physicians and support staff—to deliver exceptional care and manage the unique needs of transplant patients.

Responsibilities

Scheduling of patient evaluations, patient education, and orientation to the transplant process.

Monitoring of the evaluation process for patients, ensuring timely progression through the evaluation procedure.

Acts as a liaison for multiple hospital departments during the patient evaluation process.

Facilitates patient activities with consulting departments and physicians and communicates effectively with peers, other health‑care team members, and external contacts.

Assesses patient and family needs and communicates these needs to appropriate services—financial counseling, social work, nutrition, etc.

Provides both inpatient and outpatient care for patients throughout all phases of the transplant process.

Coordinates all post‑transplant patient care per physician orders and protocols in the outpatient and clinic setting.

Facilitates patient activities with consulting departments and physicians and communicates effectively with peers, other health‑care team members, and external contacts.

Assesses patient and family educational and other needs and directly communicates these needs to appropriate services—financial counseling, social work, nutrition, etc.

Provides both inpatient and outpatient care for patients throughout all phases of the transplant process.

Facilitates and attends weekly heart or lung transplant patient selection meetings, documenting in TITUS/UNOS all patients approved for wait listing and following up on actions identified during the weekly meeting.

Attends and participates in team meetings.

Manages logistics of donor offers for heart and/or lung transplant patients during regular working hours.

May cover donor offers after hours, holidays, and weekends on occasion.

Coordinates all teaching for pre/post heart or lung transplant patients and provides cross‑coverage to other coordinators for vacations and sick leave.

Assists with the maintenance and development of the heart and lung database(s).

Stays abreast of changes in federal, state, or local regulatory agency requirements related to transplant services and reporting.

Maintains a working knowledge of UCSF policies and procedures, follows guidelines appropriately, and contributes to the maintenance of documents.

Obtains necessary information for all registries.

Maintains communications with UNOS regarding patient listing, status changes, letters of appeal, and requests for pre‑transplant data and removal from the UNOS waitlist.

Assists financial counselors with insurance authorization by providing patient information to insurance companies for certification of treatment modalities.

Assists with the collection and participation in all supported research protocols.

Qualifications

Minimum of five (5) years of adult solid organ transplant nursing experience as an outpatient or inpatient Transplant Coordinator.

Five (5) years of RN experience in end‑stage organ‑specific disease, transplantation, and managing patients with organ failure.

Proven organizational skills and the ability to work independently, managing multiple concurrent tasks in a high‑stress environment.

Excellent communication skills, both written and verbal, and the ability to conduct oneself professionally.

Computer literacy, including proficiency in MS Word, Outlook, internet use, and a willingness to learn and adapt to new EHR systems.

Strong MS Office skills, particularly in Word, Excel, and PowerPoint, and proficiency in using databases.

Active Registered Nurse (RN) licensure in the State of California.

Active American Heart Association BLS certification.

Preferred qualifications include a BSN or MSN degree, as well as experience in assessing advanced heart failure/cardiac and/or pulmonary disease.

Clinical competency in managing transplant candidates and recipients, along with EPIC/APEX experience, is highly advantageous.

Active Registered Nurse licensure in the State of California.
