POSITION SUMMARY
As a Unit Coordinator, you will work closely with the Unit Manager, Social Services, and the interdisciplinary team to support the day-to-day administrative and operational functions of the unit. You will play a key role in coordinating communication, managing scheduling and documentation, and ensuring smooth workflows across the care team. This role is essential in supporting resident care by maintaining organized systems, assisting with admissions and appointments, and serving as a central point of contact for staff, residents, and families.

KEY RESPONSIBILITIES

Coordinates staff scheduling for the unit and prepares bi-weekly payroll accurately, in a timely manner

Functions as a receptionist for the unit taking messages and returning calls for Unit Manager, Social Worker, and Nursing staff; frequently checks and promptly responds to all emails

Transcribes Physicians’ orders; manages pharmacy refills; ensures all nursing forms are current and available

Responsible for filing, copying, and scanning/faxing documents; distributes mail and orders supplies for the unit

Creates and maintains various resident files such as disaster, personal effects, mortuary designation, annual universal lifeline forms, as well as summarizing monthly behavior monitoring log

Coordinates and completes all non- employee sitter files; partners with Human Resources to ensure background checks are completed, and works with families to complete required duties check off the list

Greets, welcomes, and interfaces with all visitors, families, residents, patients, and staff

Works closely with families and unit staff to coordinate internal/external resident appointments; prepares for and assists with new admissions; answers call lights and, participates in all quality improvement activities

Responsible for contacting families and communicating the following; appointments, bed hold notifications, information requests, and miscellaneous information on behalf of the Unit Manager, Social Worker, and IDT

Monitors residents checking in to and out of the unit

Completes online or routine catalog shopping for resident needs as directed by Social Worker and Unit Manager

Perform other duties as assigned

JOB DETAILS

Pay Rate: $27.00 per hour

Hours: Full-Time 40 hours per week

Shift Schedule: AM | 8:30 AM - 5:00 PM

Days Off: Saturday and Sunday

Working Days: Monday through Friday

REQUIRED QUALIFICATIONS

High school diploma or equivalent

One year of experience in a long-term health care setting working directly with residents, families, and staff preferred

Experience in coordination of services in a health care setting, preferred

Strong English communication skills (written, verbal, and comprehension)

Strong interpersonal skills, proven ability to handle sensitive and confidential situations

Strong analytical, organizational, time management, and problem-solving skills

Ability to work cooperatively as a member of a team

Demonstrated strong computer skills including MS Office, Excel, Word, and electronic health records

PHYSICAL REQUIREMENTS

Ability to lift, push, pull or carry a minimum of ten to fifteen (10 to 15) pounds.

Ability to stand for extended periods of time

Must be able to assist in the evacuation of residents during emergency

Ability to bend, stoop, stretch, twist, sit, and reach

Perform tasks listed above with or without the aid of mechanical devices
