See additional details below. Registered Nurse Highlights The pay for this position is competitive and based on experience . Full-time | Monday through Friday | 8:00 AM – 5:00 PM On-call rotation required once per month Onsite/community-based position This position is a contract assignment . Responsibilities Assess the health status of patients and implement individualized care plans. Provide health maintenance, disease prevention, and case management services in the home health setting. Observe, assess, plan, implement, and evaluate nursing care for well, chronic, and acutely ill patients in the community. Work closely with physicians and interdisciplinary healthcare team members to coordinate and evaluate patient care and services. Communicate changes in patient condition to the physician and complete progress reports as needed. Review, monitor, administer, discontinue, and properly dispose of medications as prescribed. Ensure compliance with medication policies related to administration, storage, errors, access, and disposal. Complete all required nursing documentation accurately, including nurse’s notes and other health-related records. Transcribe or ensure transcription of physician phone orders and medically related reports. Supervise care plans and staff members, including licensed practical/vocational nurses, as needed. Schedule and complete appointments related to health concerns and respond promptly to findings. Maintain communication with inpatient nursing staff and discharge coordinators during hospitalizations. Help maintain equipment, furnishings, and supplies in a manner that supports infection prevention. Provide training for new employees and annual recertification on health and safety topics. Train unlicensed staff on patient-specific health maintenance, safety, and disease prevention needs. Requirements Active Texas RN license required ; Compact RN license accepted . Minimum 1 year of Home Health RN experience required . Homecare Homebase experience required . OASIS experience required . Experience with wound care, disease teaching, PleurX drain, JP drain, wound vac, IV antibiotics, PICC line, ostomy care, and Foley care required . Ability to meet a productivity requirement of 30 points per week . Expected patient volume includes 4–5 patients per day and approximately 20–25 patients per week . Must be comfortable traveling throughout the San Antonio service area . Productivity Expectations Typical weekly productivity expectation is 30 points per week . Sample visit types may include: Routine Skilled Visit – 1.0 point Wound Care Complex Visit – 1.25 points Discharge Visit – 1.25 points Resumption of Care (ROC) – 1.5 points Recertification – 1.5 points Start of Care (SOC) – 2.0–2.5 points Admission with extensive teaching – 2.5 points Additional Details Mileage reimbursement is offered at $0.49 per mile . Territory includes counties in the San Antonio area . Strong preference for candidates with a well-rounded clinical background in home health nursing procedures and patient education.
