Referral Co‑ordinator (Fixed Term – 6 months, Part Time – 18.5 hours/week)
Bristol City Council is seeking a confident and organised Referral Co‑ordinator to join our Bristol Reablement Service. This is a key frontline role, acting as the first point of contact for referrals into the service and ensuring people receive the right support at the right time to help them regain independence.

About The Role
As a Referral Co‑ordinator, you will play a vital part in ensuring referrals are assessed, triaged and progressed efficiently and safely.

What You Will Do

Act as the first point of contact for all referrals to the Reablement service.

Assess referrals within agreed timescales, identifying risks and gathering relevant information from electronic systems.

Make initial decisions on eligibility and suitability for Reablement.

Clearly explain decisions where referrals do not meet eligibility criteria and signpost to appropriate alternative services.

Allocate staff using service‑specific IT systems to ensure assessed needs are met.

Ensure all required documentation is completed before services begin.

Maintain accurate and up‑to‑date electronic records.

Use staff time and resources efficiently to support effective service delivery.

Work collaboratively with colleagues and promptly report any rostering or capacity issues.
