Overview
Accepts assignments in Palmdale, Antelope Valley, and Poppy Valley clinics.

Purpose and Scope
Functions as part of the dialysis health care team. Provides safe and effective dialysis therapy under direct supervision of a licensed nurse, following organization policies, procedures, and regulatory requirements.

Responsible for setup and operation of hemodialysis machines, maintenance of a safe and clean environment, and support of the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities.

Principal Duties and Responsibilities
Patient Related

Assist other health care members in providing patient‑specific education on adequacy measures (OLC, AMP, UKM) and disease process/access.

Welcome assigned patients and inquire about their wellbeing since last treatment.

Evaluate vascular access for patency, cannulate, and administer heparin as delegated.

Obtain pre‑ and post‑treatment vital signs, weight, and perform access evaluation.

Initiate dialysis treatment according to prescribed orders (QB, QD).

Evaluate intradialytic problems and provide intervention per physician order or direction.

Monitor patients’ response to dialysis therapy.

Obtain vital signs prior to reinfusion and discharge patients, document all actions.

Discontinue dialysis treatment per procedures, evaluate patient after venous access removal.

Obtain hemostasis and apply appropriate dressings.

Perform and record pre‑ and post‑dialysis evaluations, weight, and vital signs with identification.

Calculate and enter individual patient dialysis programming for UF goal, treatment time, and UF modeling.

Ensure safety alarms are enabled (Narrow Venous Limits 160).

Record accurate vital signs and treatment parameters on the Hemodialysis Treatment Sheet.

Document patient information, complete Hemodialysis Treatment Sheet and progress notes.

Report significant changes in patient condition to the RN or supervisor.

Observe patients, perform machine safety checks per policy, report unusual findings.

Perform and document interventions for unusual patient status and outcomes.

Monitor vascular access pressures, provide appropriate interventions, report findings.

Obtain all prescribed laboratory testing and prepare specimens.

Clean blood spills immediately with disinfectant per policy.

Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.

Assist patients in self‑care under RN supervision.

Staff Related

Assist in training applicable direct patient care staff on programs under supervision of a nurse.

Education/Communication

Follow all organizational policies, procedures, and systems in training.

Maintain environmental integrity and aesthetics; ensure safety and cleanliness.

Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments.

Conduct all tasks necessary for preparation for dialysis treatment and perform pretreatment alarm testing (PHT).

Initiate Solution Delivery System (SDS).

Record Keeping

Complete and document ongoing CQI activities.

Enter all treatment data into the designated clinical application accurately and timely.

Review treatment sheets for completeness, ensure nursing signatures are documented, and correct omissions.

Prepare initial patient chart for admission and compile paperwork.

Prepare/print lab requisitions per laboratory destination.

Collect, label, appropriately prepare, and store lab samples per specifications.

Ensure collection of lab specimens by appropriate courier.

Infection Control

Assist in collecting information for infection control audits.

Support staff and patient adherence to infection control practices.

Follow infection control policies and procedures.

Participate in and reinforce infection control education under direction of CM or designee.

Transitional Care Unit

Follow newly admitted patients through first four weeks of dialysis and coordinate transitional care.

Reinforce education and care matters as allowed by state law.

Liaise with FKC staff to provide optimal patient experience and scheduled visits.

Assist in patient understanding of home dialysis products and benefits.

Set and perform treatments on all machines in the transitional care unit.

Assist in demonstrating peritoneal dialysis set‑up.

Assist with coordination of transition to patient’s modality choice.

Assist with supply ordering and inventory for TCU.

Physical Demands and Working Conditions

Perform desk and computer work, interact with patients, staff, and physicians.

Travel between assigned facilities and community locations; travel to regional, Business Unit, and Corporate meetings may be required.

Work environment characteristic of a health care facility with air control and moderate noise.

Direct patient care involves heavy lifting and patient ambulation; assistance from coworkers may be provided.

Requires frequent, prolonged periods of standing and the ability to bend over.

May be required to push/pull equipment, exert up to 15 pounds of force, lift solutions up to 30 lbs. (occasionally 40 lbs.) up to 5 ft.; two‑person assist and material assist devices available.

Education and Licenses

High School diploma or GED required.

Must meet CMS‑approved state and/or national certification requirements.

All appropriate state licensure, education, and training (if any) required.

Demonstrated commitment to organization culture, values, and customer service standards.

Experience and Required Skills

Previous patient care experience in a hospital setting or related facility preferred.

Successful completion of the organization dialysis training program and CPR certification required.

Compensation and Benefits
Hourly Rate: $24.00 – $30.00.
