Essential Functions
Performs daily tasks of vital signs, blood sugar testing, intakes & outputs, EKGs, and assisting consumers with activities of daily living (i.e. baths, positioning, feeding) under the direction of a licensed nurse
Provides ongoing monitoring and interaction with the consumers
Provides daily assessment of consumer’s behavior and records and submits information according to policy and standards
Assists consumers with personal care, activities, current events and safety issues
Provides training on basic living skills according to the consumer’s psychiatric rehabilitation goals
Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
Effectively collaborates with the treatment team member
Demonstrates willingness to work on any unit at BayPointe/EastPointe based on facility needs
Models appropriate behavior including but not limited to communication skills, grooming, manners, etc.
Meets AltaPointe productivity standards
Provides one-to-one coverage to consumers on special status as assigned
Performs additional duties as assigned by supervisor
Reports accident or driving violations to supervisor and human resources
Accepts and employs suggestions for improvement
Actively works to enhance skills
Documents in a timely fashion per AltaPointe policy
Documents in a clear and concise manner the types of consumers’ problems, daily progress notes, progress on BLSs, treatment goals addressed, and services provided
Actively participates in performance improvement activities
Actively participates in AltaPointe committees as required
Follows AltaPointe policies and procedures
Attends appropriate in‑service trainings and workshops
Seeks supervision and consultation as needed
Documents legibly
Documents activities performed
Treats consumers with care, dignity and compassion
Respects consumers’ privacy and confidentiality
Is pleasant and cooperative with others
Assists consumers and visitors as needed
Personal values don’t inhibit ability to relate and care for others
Is sensitive to the consumers’ needs, expectations and individual differences
Is gentle and calm with consumers and families
Physical Requirements
Frequent standing, sitting and walking in bursts of 5 to 10 minutes; may be extended with the ability to change positions as necessary
Occasional forward bending from the hips while kneeling to reach to the side
Occasional full squat/lunge from a standing position
Occasional maintenance of back extension while on stomach with body weight on hips and legs for several minutes
Occasional ability to place one’s own body weight on one knee and one elbow on the same side of the body
Occasional kneeling and reaching to same side toward the floor
Lift up to 10 pounds from the floor to various levels, including overhead, occasionally during shift
Lift up to 50 pounds to assist in activities of daily living, from floor to waist, as required
Strong grasp needed to use keys to open doors throughout the facility, and to maintain constant contact with a consumer during a restraint procedure or other hold techniques
Pushing/pulling force up to 25 pounds occasionally during shift
Ability to ambulate quickly to assist coworkers with a consumer in a restraint procedure (e.g., jog/run up to 0.1 miles)
Activities may include therapy inside or outside the facility; some tasks will dictate physical requirements for playing or games
Completes assigned tasks in a timely manner
High School Diploma or GED and current Certified Nursing Assistant in Alabama required.
