Bring hospital-level care to patients—right in the comfort of their homes. We are seeking experienced and compassionate Registered Nurses to join our innovative Hospital-at-Home program.
Position Overview:
As a Home Care RN, you will deliver skilled nursing visits to patients who are formally admitted to the hospital but receiving care at home. You’ll play a vital role in providing high-acuity, patient-centered care while working closely with a coordinated care team and real-time command center support.
Key Responsibilities:
Perform comprehensive head-to-toe assessments
Administer IV infusions and manage infusion therapy
Provide wound care and dressing changes
Conduct lab draws and specimen collection
Monitor patient condition and respond to clinical changes
Communicate patient status updates by phone to a centralized command center during visits
Collaborate with a team of nurses to ensure seamless, around-the-clock patient care
Scheduling Requirements:
Must be available for morning, midday, and evening visits
Daily patient care may be shared among a team of nurses to support continuity and coverage
Qualifications:
Active RN license in good standing
Experience in acute care, home health, or infusion therapy preferred
Strong clinical judgment and critical thinking skills
Excellent communication and documentation abilities
Reliable transportation and valid driver’s license
