MSW
Care Manager
to support our elder clients in Tinton Falls, NJ, and surrounding areas.
This role requires the ability to assess, plan, and coordinate care for older adults both at home and in facility settings, while advocating for their independence and well‑being.

Responsibilities
Conduct comprehensive assessments and develop individualized care plans
Coordinate home care services and other supports
Attend medical appointments as a client advocate and provide ongoing support
Navigate complex healthcare systems to secure appropriate services, resolve barriers, and expedite access to care
Act as a liaison for families at a distance
Arrange, monitor, and adjust in‑home services as needed
Provide crisis intervention and problem‑solving support
Engage and coordinate community resources to enhance clients’ quality of life
Promote meaningful social, spiritual, and cultural activities
Maintain accurate records and documentation of client care
Qualifications
Masters of Social Worker (MSW)
required
Experience in geriatrics or elder care preferred
Strong interpersonal and communication skills
Ability to work independently while collaborating with families and service providers
Valid driver’s license; company car provided for work‑related travel

Monday‑Friday 8:30‑5pm hours, after‑hours client support availability needed
