As a Licensed Practical/Vocational Nurse, you will provide direct, compassionate nursing care to hospice patients under the supervision of a Registered Nurse. You play a critical role in managing comfort, supporting families, and ensuring every encounter reflects our values—trustworthy, capable, compassionate, proactive, and called. This role is essential to delivering dignified, high-quality end-of-life care.
Essential Functions
Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end-of-life care.
Implement current nursing practice following a comprehensive assessment and the established plan of care.
Assess and educate patients and their families/caregivers on diagnosis, care needs, and symptom management.
Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.
Deliver appropriate pain and symptom management; monitor patient response to treatments and medications.
Communication & Coordination
Document all care accurately and timely in alignment with the plan of care.
Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.
Additional Responsibilities
Participate in ongoing personal and professional development.
Contribute to quality assessment and performance improvement (QAPI) activities.
Perform other duties as assigned by the Registered Nurse.
Qualifications
Graduate of an accredited practical nursing or vocational nursing program.
Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal.
Current state LPN/LVN licensure.
Current CPR certification.
Demonstrated adherence to professional nursing standards and practices.
Licensed driver with a reliable, insured automobile meeting organizational requirements.
