Overview
The setting for this position is Independent Living, Assisted Living and Memory Care.

Provide Speech‑Language Pathology Therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of Speech‑Language Pathology therapy and within Reliant Rehabilitation guidelines. Provide quality patient care including screening and evaluation of patients, development of initial and ongoing treatment plans of care, discharge plan, supervision of assistants and Rehab Techs, and case management responsibilities. Maintain professional conduct that represents Reliant Rehabilitation’s commitment to quality and service in all interactions with clients, including positive levels of interaction and rapport with facilities and clients. Adhere to all state and federal regulations regarding speech‑language pathology therapy.

Responsibilities

Optimize communication, cognitive‑linguistic abilities, and swallowing skills of patients by providing, directing, and/or overseeing speech therapy. Responsibilities include screening of residents, evaluating and treating patients referred by a state‑approved referral source, and ensuring accurate and timely billing and documentation in the medical record.

Establish and maintain effective interactions with patients, the interdisciplinary team, and other clientele reflecting patient‑centered care.

Directly supervise Rehab techs in group and individual treatment tasks to promote efficient utilization of resources and facilitate ongoing development of clinical skills in accordance with State Practice Acts, OBRA, and Reliant Rehabilitation policy.

Build a strong Speech‑Language Pathology (SLP) therapy program by: actively integrating SLP therapy’s role into the community and interdisciplinary treatment team; contributing to community patient care, utilization review, case management, administrative staff/department head, and family conference activities and communications; establishing and maintaining good working relationships with other health‑care professionals and colleagues; making appropriate referrals to other members of the rehabilitation team; developing and implementing new programs/techniques to enhance rehabilitation services; adhering to Reliant Rehabilitation and client‑specific policies and procedures related to environmental safety, orderliness, risk management, infection control, and emergency preparedness; and following patient rights and privacy requirements.

Enhance clinical expertise, professional and managerial skills through interaction with managers, therapists, and other professional staff, self‑study, and continuing education activities.

Maintain a valid state license (where applicable) and stay aware of issues related to the profession of speech‑language pathology therapy and the healthcare environment.

Understand relevant payment models for billing and treatment guidelines, and provide quality care within reasonable goals and outcomes.

Be compliant with infection control procedures and environmental safety protocols within a community.

Requirement to lift patients using appropriate biomechanical techniques; must be capable of lifting thirty (30) pounds of dead weight alone.

Maintain confidentiality of Reliant Rehabilitation, community, and patient information.

Conduct oneself in an ethical, legal, and responsible manner at all times.

Adhere to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct.

Be able to travel between facilities during scheduled hours.

Work varying hours and shifts, including weekends and holidays.

Attend all mandatory meetings, trainings, and assignments as delegated.

Perform other duties and responsibilities as assigned.

Qualifications

Prior to beginning patient care, a peer SLP may complete a Competency Checklist.

Effective clinical problem‑solving skills: identify and prioritize issues, develop sound and innovative action plans, execute plans timely, and ensure appropriate results.

Strong team‑player attitude.

Strong verbal and written communication skills.

Ability to build relationships with a diverse population.

Fluent in English: read, write, speak, and comprehend instructions, correspondence, charts, memos, and reports.

Ability to work in a fast‑paced, productive environment.

Education / Experience
Master’s Degree, or equivalent, in Speech Language Pathology.

Computer Skills

Proficient knowledge of Microsoft Office (Outlook, Word, Excel, PowerPoint, etc.).

Basic Internet navigation skills.

Certificates and Licenses

Valid Speech‑Language Pathologist state license (where applicable).
