Care Coordinator Position - StepOne Service (offered by Evergreen Healthcare Partners, LLC)
StepOne Service is an inpatient, hospital-based 3 - 5 day Acute Withdrawal Management service available to adults requiring medical management of withdrawal symptoms from alcohol and/or opioids.

Acute Withdrawal Management is provided to individuals who are in impending or early stages of acute withdrawal at the time of hospital admission. The goal is to medically assist the individual through the withdrawal process and help navigate the transition to an appropriate level of community-based addiction treatment that can address the underlying causes of their substance use disorder.

StepOne Service has a fully qualified treatment team including an attending physician and nursing staff that utilize a multidisciplinary team approach in the acute management withdrawal of patients.

Position Purpose
The Care Coordinator is responsible for the management of the StepOne Service office, which includes managing telephone inquiries concerning StepOne Service, screening potential patients, assisting with patient questions and concerns and initiating discharge planning. The Care Coordinator coordinates the service within the hospital and functions as a resource to the nursing staff, unit supervisors (and/or other identified hospital staff), and the EHP leadership team to coordinate a continuum of care.

Essential Functions

Effectively screening, verifying and reporting patient information

Effectively obtains and manages all information related to criteria required for admission, continued stay and discharge

Establish resources needed to document and implement patient discharge plans by networking inside the hospital and by phone with outside sources

Provide support to the StepOne Service Service Coordinator and hospital contacts to ensure optimal success of the service

Typical Experience/Qualifications

Basic understanding of substance use disorders

Knowledge of all related computer applications

Understanding of social services and/or hospital procedures and policies

Understanding of HIPAA guidelines and policies

2+ years of social service experience and/or clinical or hospital inpatient services

Experience with telephone networking/outreach preferred

Prior experience with substance use disorder population

Work history as a results oriented team player willing to accept and promote organizational goals

License/Certified Credentials in the area of social services

Ideal candidate will have associated experience and/or a bachelor’s degree in the related field

Experience in a self-motivated role
