Overview
Join CenterWell Home Health as an LPN and work with a dedicated team of physicians and clinicians to provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. You will manage care plans that support recovery and help patients get back to the life they love.
Key Responsibilities
Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
Implement individualized care plans in collaboration with the patient, family, and healthcare team.
Educate patients and their families on disease management, treatment options, and self‑care techniques.
Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
Maintain contact with patients, physicians, clinical manager, and other members of the healthcare team promptly regarding patient or family needs and status changes.
Participate in care coordination activities and discharge planning.
Required Qualifications
Current nursing license in the practicing state.
Valid driver’s license, auto insurance, and reliable transportation.
Current CPR certification.
Two years experience as an LPN/LVN in a clinical setting.
Preferred Qualifications
Nursing experience in a Home Health or Hospice setting.
Pay Range
$37.00 – $52.00 pay per visit/unit.
$58,300 – $80,300 per year base pay.
Additional Information
This role is patient‑facing and is part of Humana's Tuberculosis (TB) screening program. Screening for TB is required if selected.
This role is part of Humana's driver safety program and requires a valid state driver’s license and maintenance of personal vehicle liability insurance. Insurance must meet state minimum limits or $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 property damage (or the higher amount).
Scheduled weekly hours: 40.
