About the Role
Touchstone Health is seeking a
full-time Hospice Social Worker
in San Antonio who is called to walk alongside patients and families during some of life’s most meaningful and vulnerable moments. In this role, you’ll provide compassionate, high-quality psychosocial care that supports not just clinical outcomes—but dignity, understanding, and peace.
You’ll primarily serve hospice patients, while also supporting
home health patients
as needed, offering a broader scope of impact across the continuum of care. You’ll be part of a collaborative team grounded in trust, empathy, and shared purpose. Here, your work goes beyond coordination—you help bring clarity to complex situations, advocate for patient needs, and bring out the courage in others when it matters most.
Key Responsibilities
Provide social work services to hospice patients in accordance with the physician’s plan of care
Support home health patients as needed, ensuring continuity of psychosocial care across settings
Support patients and families in navigating emotional, social, and environmental challenges
Connect patients to community resources, financial assistance, and support services
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Assist with discharge planning and care transitions to ensure continuity and safety
Document assessments, interventions, and care coordination accurately and in a timely manner
Requirements
What We’re Looking For
Master’s degree in Social Work (MSW) from a CSWE-accredited program
Active licensure as required by the state of Texas
At least one year of social work experience in a healthcare setting (hospice experience preferred)
Strong skills in communication, assessment, and patient advocacy
Deep empathy and the ability to navigate complex family dynamics with sensitivity and professionalism
Detail-oriented, self-directed, and comfortable working independently in a field-based role
