Position Summary
The Physical Therapist is responsible for assuring that the PNHS values of Head & Heart are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, outcome/compensatory assessments, compliance with regulatory standards, and participation in process improvements to advance patient care outcomes and satisfaction.
The Physical Therapist will communicate in a timely, clear, and professional manner with staff, patients, patient family members, physicians, and other internal/external customers. Therapists will demonstrate skills in listening, conflict management, system problem solving, decision making, and process improvement while practicing in compliance with professional standards of practice and the professional code of ethics.
The Homecare Physical Therapist acts as a case manager to assess and evaluate patient care needs, providing skilled therapy care and teaching to patients and families in their homes. The therapist is responsible for improving and preparing patients and families for self‑directed and/or assisted care in their community settings. Coordination of medical, social, and financial resources is performed to develop a comprehensive plan of care, supporting community care processes such as admission, discharge, referrals to other disciplines, billing, and care delivery.
Work Schedule
Monday – Friday 8:00 am – 4:30 pm, one day off per week; occasional weekends.
Location
Central Region: St. Louis Park, Minneapolis, Hopkins, Golden Valley, Edina.
Required Qualifications
Bachelor's, Master's, or Doctorate degree in Physical Therapy
Current license with Minnesota Board of Physical Therapy
Knowledge, skills, and abilities:
Demonstrates knowledge of age‑related growth and development principles to provide appropriate service regarding primary diagnosis and multiple comorbidities affecting care.
Proficiency with communication and data processing equipment, clinical apparatus related to patient care in homecare, electronic medical record systems, and OASIS evaluation and data sets.
Preferred Qualifications
Work experience as a Physical Therapist for at least 3 years preferred.
Experience in homecare desirable with knowledge of OASIS data set.
High level assessment skill including medication reconciliation and teaching, technical skill, and communication skill.
Demonstrates initiative, judgment, critical thinking, and ability to problem‑solve independently.
