About the Role
Cleveland Clinic Fairview Hospital, a Magnet Hospital, is recruiting a Community Health Worker (CHW) to work with the Chronic Disease population.

Responsibilities

Establish trusting relationships with patients and their families, providing support, encouragement, and feedback.

Coach patients in effective management of their chronic health conditions and self‑care.

Convey program purposes and services to the community and explain impacts.

Assist patients in understanding care plans and instructions.

Visit patients in their homes as needed to assess living conditions and meet with family.

Document activities, service plans, and results, strictly adhering to policies and procedures.

Advocate for patients and act as liaison between patient/family and community service agencies.

Help patients utilize resources, schedule appointments, and finish applications for eligible programs.

Assist patients in accessing health‑related services, including obtaining a medical home and overcoming barriers to care.

Facilitate communication and coordinate services between providers.

Motivate patients to be active, engaged participants in their health.

Work effectively with staff, clients, doctors, agencies, and others from diverse backgrounds to reduce cultural and socioeconomic barriers.

Build and maintain positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors, and office staff.

Continuously expand knowledge of community resources, services, volunteer programs, and departmental procedures.

Minimum Qualifications

High‑school diploma or GED.

Valid driver’s license.

Knowledge of community resources.

Community Health Worker (CHW) certification and be certified as a CHW through the Board of Nursing.

Ability to develop trusting and effective working relationships.

Strong information‑gathering, interviewing, and time‑management skills.

Preferred Qualifications

One year of experience in a medical office setting.

Two years of community service experience.

Bilingual in English and Spanish.

Medical Assistant training or certification.

Knowledge of medical terminology.

Knowledge of the Electronic Medical Record (EMR).

Physical Requirements

May need to move around intermittently during the day: sit, stand, stoop, bend, ambulate, remain still for extended periods.

Work evenings and weekends may be required.

May need to lift boxes or carry objects.

Possess dexterity to operate a PC and other office equipment.

Personal Protective Equipment

Follow standard precautions and use personal protective equipment as required.
