Job Overview
The Population Health Clinical Manager will lead the development, implementation, and management of population health initiatives. This role will oversee clinical operations, ensure the delivery of high quality care and utilize data analytics to drive improvements in health outcomes for targeted populations. The ideal candidate will have a strong clinical background and experience in population health management.
Required Qualifications
Bachelor's degree in nursing, public health, or a related field
Minimum of 3 years of experience in clinical operations, population health, or healthcare management
Strong knowledge of healthcare systems, patient care models, and population health metrics
Proven leadership skills with the ability to motivate and guide a diverse team
Excellent analytical, problem solving, and decision-making skills
Effective communication and interpersonal skills with the ability to work collaboratively in a team environment
Proficient in data analysis tools and electronic health record systems
Certifications and Licenses Required
NJ RN license required
Certification in population health management or related field preferred
Preferred Qualifications
Masters degree preferred
Essential Job Functions
Develop and implement population health strategies that align with organizational goals and enhance patient care
Oversee clinical programs focused on disease management, preventive care, and health promotion for diverse populations
Collaborate with interdisciplinary teams including physicians, nurses, social workers, and community partners to create integrated care plans; utilize data analytics to identify trends in health outcomes, patient engagement, and resource utilization and implement strategies for improvement
Monitor and report on key performance indicators related to population health initiatives
Ensure compliance with regulatory and quality standards in all population health activities
Conduct training and education sessions for clinical staff on best practices in population health management
Facilitate community outreach efforts to engage and educate patients on available resources and health initiatives
Act as a liaison between clinical teams and leadership to advocate for patient-centered care practices and policies
