Position Summary
We’re looking for a committed, professional full‑time
Patient Navigator
for our Manhattan location. The ideal candidate is personable, works well with a diverse population, can communicate effectively with medical providers and support staff, and has good attention to detail. The Patient Navigator provides field‑based case management services to patients enrolled in the HIV care coordination program. The navigator is the main point of contact for the patient, building strong relationships with HIV‑positive patients to help them connect and stay engaged in prevention and treatment services. The patient navigator removes barriers to care by identifying critical resources and helping patients navigate health‑care services and systems, promoting their health. The navigator works closely with the care team (doctors, nurses, and other staff) to ensure optimal patient outcomes.
Job Responsibilities
Provide direct services to people living with HIV/AIDS under the supervision of a Care Coordinator.
Carries out tasks needed to execute medical and support service plans, including accompaniment to appointments, coaching, health‑education encounters, entitlements reassessment, logistics coordination for plan adherence reminders, transportation, and childcare arrangement.
Conduct field‑based tasks: observe and record patient self‑administration of ARVs, assess and report any ARV‑related side effects, and deliver visits at home, CBO, work, or any convenient location for PLWHA.
Maintain client records and required documentation according to APICHA’s Client Services Department protocols and standards.
Participate in unit quality assurance and continuous quality improvement (CQI) tasks.
Represent the agency at various venues to promote services.
Perform other assigned activities.
Qualifications
Associate’s degree in human services or social work and at least 12 months of experience in patient navigation or a similar field.
Certification as a peer worker by the NYS AIDS Institute Peer Certification, case‑management certification by CUCS, or at least 90 hours of documented training equivalent to NYS Peer Worker requirements.
Strong socio‑cultural identification with the target population or experience working with them.
Fluency in one or more Asian languages preferred.
Excellent written and verbal communication skills in English required.
Proof of immunization or immunity to certain communicable diseases (including influenza during flu season) and tuberculosis testing, as required by NYC DOHMH Health Code, NYSDOH, and OSHA.
