Nurse Practitioner, Clinical Instructor, Internal Medicine, General & Geriatric

Stony Brook Medicine's Nurse Practitioners provide clinical services in a primary care general medicine/geriatric office. Candidates will work full-time under the supervising physician and demonstrate superior patient care.

Responsibilities

Perform urgent care visits, follow-up office visits, and transitional care management visits as needed.

Document all visits into the electronic medical record in a timely fashion.

Perform or learn comprehensive geriatric assessment, including functional, cognitive, depression screens, gait and balance assessments.

Perform Annual Medicare Wellness Visits.

Review laboratory, radiology, pathology, cardiac and other testing results with patients.

Refill prescriptions and prescribe new medications as needed.

Triage patient messages and cover electronic medical records inbox when needed.

Coordinate follow-up care, including consultation referrals, ordering diagnostic tests, arranging home care services, durable medical equipment, skilled nursing facility/subacute rehab, ED transfer and/or inpatient admission when necessary.

Perform relevant outpatient procedures (e.g., cerumen removal) as clinically indicated and permitted by practice agreement.

Participate in clinical research programs and quality improvement initiatives within the Division of General Internal Medicine, Geriatrics and Hospital Medicine.

Perform other duties as assigned by collaborating physician, medical director or division chief.

Required Qualifications

NYS license and certification as a Nurse Practitioner.

Master's Degree (foreign equivalent or higher) in Nursing or completion of a course for a Nurse Practitioner and certification in Adult/Geriatric care. Certification in Family Medicine with ambulatory experience also acceptable.

One year full-time working experience in Adult or Geriatric care.

Preferred Qualifications

Additional years of Geriatric or Primary Care internal medicine experience.

Previous experience in ambulatory care.

Motivation, dedication and flexibility.
