Education
Minimum: Nursing Diploma or Associate's
Preferred: Bachelor's Degree in Nursing
Experience
Minimum: 2–5 years ICU RN
Preferred: 5–10 years ICU RN
License & Certification
Registered Nurse (RN) license in Illinois
Responsibilities
Facilitate identification and resolution of gaps in care from hospitalization to discharge and into ambulatory settings.
Lead development and implementation of complex individual plans of care, collaborating with interdisciplinary team, patient, and family.
Apply science-based practice, participate in journal club, evaluate evidence-based practices to improve care.
Analyze practice standards and provide feedback to peers and leadership.
Lead evaluation of patient and family experience to improve satisfaction and quality of care.
Work safely as a safety-sensitive position requiring constant state of alertness.
