
Public Health Specialist II - Limited Service

The You First team is seeking a dedicated Lifestyle Program Specialist to support our heart health initiative. This role involves working directly with program members to enhance their health and reduce the risk of cardiovascular disease through personalized support and resources.
Responsibilities
Review cardiovascular risk assessments (blood pressure, A1C, cholesterol, health history, and social determinants of health)
Utilize motivational interviewing techniques
Provide personalized health coaching to support lifestyle changes
Facilitate access to resources such as fresh produce (farmers markets/CSAs) and physical activity opportunities (walking programs, gym memberships, exercise classes)
Manage relationships with state vendors.
Perform documentation and administrative tasks.
Member interaction is primarily conducted by phone and email. This position is funded by a cooperative agreement with the Centers for Disease Control and Prevention. This is a hybrid position based in Waterbury, VT.
Minimum Qualifications
Three (3) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
OR Associate degree in public health, public administration, business administration, environmental science, emergency response, human services, social work, social sciences related fields or health-related fields and two (2) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
OR Bachelor’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields and one (1) year or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
OR Master’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields.
OR One (1) year or more of experience as Public Health Specialist I.
