Overview
We are Catholic Community Services, Family Behavioral Health, offering family‑oriented behavioral health care for children, youth and families primarily in their homes and community settings. We are looking for a Mental Health Care Coordinator (Internal title: Clinical Care Coordinator) to provide community‑based case management in the Vancouver, WA / Portland Metro area (Oak View Site – Near the Vancouver Mall).
Location
Oak View Site, Vancouver, WA near the Vancouver Mall. Services are provided in community‑based settings and in clients’ homes.
Responsibilities
Meet and engage families with compassion, support, respect and enthusiasm for the role of helper.
Provide and/or arrange necessary crisis response and stabilization services, completing and communicating safety/crisis plans.
Prepare families for next steps in the intervention, including team development, planning, and natural supports.
Customize helping approaches to fit each family’s uniqueness, personality, culture and interests.
Provide intensive, strengths‑based therapeutic services to children, youth and families, including safety assessment, treatment planning, crisis planning, and facilitation of child and family team meetings.
Brainstorm and negotiate strategies that build on child and family strengths.
Consult with clinical supervisors and mental health specialists and incorporate recommendations.
Meet productivity and documentation standards with complete, accurate, strength‑based documentation.
Respond to crises in a prompt and collaborative manner.
Provide and facilitate individual and family therapies as part of the overall plan.
Utilize parent/family expertise in problem‑solving around specific needs.
Modify and adjust intervention techniques without changing the overall direction of the plan.
Facilitate team implementation of the plan from beginning to end.
Reevaluate, modify and redesign the plan based on new information.
Inspire confidence in the child, family and team members about their strengths and ability to transition successfully.
Arrange and negotiate ongoing formal and informal services and supports.
Develop a plan for aftercare supports and response after CCS involvement.
Participate in an on‑call rotation with other clinicians and coordinators.
Qualifications
Bachelor’s degree in social work, psychology, behavioral sciences or an equivalent accredited degree.
Demonstrates attitudes, knowledge and skills to deliver culturally competent services in multi‑cultural settings.
Energetic, innovative style that engages children, youth and families.
Enthusiastic and optimistic, encouraging ongoing participation of all involved.
Natural strengths‑based approach in working with others, appreciating unique qualities and assets.
Passion for helping families stay together.
Ability to work independently and as part of a team.
Ability to visually assess safety within family homes.
Ability to meet clients in homes and community locations.
Reliable transportation, valid driver’s license and automobile insurance with acceptable driving record.
Preference for a flexible schedule over a traditional 8‑5, Monday‑through‑Friday, facility‑based position.
Willingness to participate in an on‑call rotation as discussed in the screening process.
Successful completion of required background clearances.

Compensation & Benefits
Start Pay Range: $33.06 – $36.78 per hour.
Additional Pay: First Responder $1,800 per year; performance‑based up to $8,000 per year.
Training: Extensive training in multiple clinical approaches and other areas.
Supervision: Weekly supervision and daily support with a Clinical Supervisor.
Paid holidays: 12; plus 1 personal holiday per year.
Vacation: 3 weeks per year.
Sick days: 12 per year.
Medical, dental and vision insurance.
Life insurance equal to one annual salary.
Retirement plans: 403‑B Employee Savings Plan and employer‑contributed pension.
