Responsibilities
As a Pediatric Home Care RN or LPN, you will work one‑on‑one with children and families to develop a care plan tailored to their needs. You will provide skilled nursing care to pediatric‑young adult patients with special health care needs in the patient’s home. Duties include patient assessments, feedings, respiratory system management, medication administration, activities of daily living, and therapy programs. You will also assist in care coordination with the care team and may be responsible for patient transportation.
Qualifications
Graduate of a State Board approved program for Licensed Practical Nurse or Registered Nurse and hold a valid license.
Strong computer skills.
Ability to understand and apply guidelines, policies and procedures.
Maintain an acceptable driving record (no OWI convictions in the last 7 years and no more than 3 moving violations in the last 3 years). An unacceptable driving record may result in suspension, termination, or transfer to a position that does not require driving.
Ability to work as a collaborative team member.
CPR: Maintain a valid Basic Life Support Healthcare Provider Card with re‑certification.
