
Community Health Worker l

Job Overview
This role can be based out of either MercyOne in Clinton, IA or MercyOne Genesis in Davenport, IA. The position is full time. It serves as a bridge between the health system and patients, particularly those with Medicare and/or Medicaid and identified social needs.
Essential Functions
Knows and demonstrates the Trinity Health Mission, Vision, and Values in daily practice.
Delivers high‑value services such as coordination of care, accompaniment to appointments, outreach, support, advocacy and referrals.
Assesses patients for social needs, barriers, and helps them access health care and community resources.
Provides basic education on health promotion and shares patient cultural insights with providers.
Uses multiple systems for caseload management, data collection, reporting, and electronic health records.
Communicates effectively, builds trust, and serves as a liaison between providers and patients.
Maintains a safe, productive workspace, stewards resources, and supports team member growth.
Performs additional duties as assigned by the supervisor.
Keeps working knowledge of applicable laws, policies, and Trinity Health’s integrity program.
Minimum Qualifications
High school diploma or GED and at least one year of community involvement or volunteer experience.
Comprehensive knowledge of the lived experiences and cultural background of the patient community.
Completes CHW training/certification within 6 months of employment and maintains certification per guidelines.
Comfortable in a collaborative, shared leadership environment.
Demonstrates honesty, integrity, caring, and the ability to inspire others to promote the philosophy, mission, vision, goals, and values of Trinity Health.
Physical and Mental Requirements
Set and organize own work priorities; adapt to changes and handle multiple tasks.
Frequent clinical or patient‑facing work; continuous indoor work with variable external conditions.
Occasional lifting up to 30 pounds and extensive walking, standing, pushing, and pulling.
Must travel to MercyOne sites as needed and comply with policies and procedures.
