St. Vincent de Paul Louisville (SVDP) is a mission-driven organization dedicated to providing housing, food, and support services to individuals and families in need. The organization offers a continuum of care, addressing issues such as homelessness, poverty, addiction, and mental illness to help individuals achieve self-sufficiency. SVDP operates shelters, supportive housing, a community kitchen, a food pantry, youth programs, and thrift stores, serving up to 600 individuals and families both on campus and in the community. Role Description
This is a full-time on-site role for a Case Manager located in Louisville, KY. The Case Manager will be responsible for working directly with program participants to provide comprehensive case management services. Responsibilities include conducting assessments, creating individualized care plans, connecting participants with community resources, assisting in goal setting, and advocating for clients. The role also involves maintaining accurate and timely client records, ensuring compliance with program requirements, and collaborating with other staff to address the diverse needs of clients.

Qualifications

Strong interpersonal, communication, and active listening skills with an ability to build trusting relationships.

Experience in conducting assessments, developing care plans, and executing case management strategies tailored to client needs.

Understanding of mental health, addiction, housing, and poverty-related issues to provide empathetic and effective client support.

Organizational and documentation skills to ensure accurate and timely record-keeping and compliance with program requirements.

Familiarity with community services and the ability to connect clients with appropriate resources and support programs.

Bachelor’s degree in Social Work, Psychology, or a related field; relevant work experience may also be considered.

Proficiency in Microsoft Office Suite and case management software is advantageous.

Experience working in non-profit, social service, or community-focused organizations is a plus.

Cultural competency and commitment to working with individuals from diverse backgrounds.
