Responsibilities
Position Summary: The Social Worker/Case Manager is responsible for managing assessments and discharge planning activities offered to the patient, which may include assessing patients’ needs and compiling a treatment plan within a multidisciplinary team; planning and/or providing care, treatment, and services; collaborating and exchanging information with the treatment team, external providers, collateral contacts, and managed care organizations; and implementing the discharge planning process including a thorough risk assessment and arranging follow‑up care and resources. The Social Worker/Case Manager will work closely with a Masters‑level clinician to provide oversight on assessments and clinical consultation as needed.

Qualifications

Education: Bachelor’s Degree from an accredited college or university in social work, counseling psychology, mental health, or a related field. Enrollment in a Master’s Degree program in Social Work or Counseling Psychology preferred.

Experience: One (1) year direct clinical experience in a psychiatric or mental health setting preferred. Experience in patient assessment, risk and safety assessments, treatment planning, community resources, family systems and communication, psychiatric diagnoses and effective treatments, and coordination with external review organizations preferred.

Additional Requirements: CPR certification and successful completion of Crisis Prevention Intervention (CPI) training (may be obtained during new hire orientation).

Primary Responsibilities

Assessments

Complete thorough assessments and documentation within allotted timeframes, including patients’ behavior, criteria for hospitalization, and risk assessment.

Use hospital criteria for continued evaluation for hospitalization and make appropriate recommendations for level of care upon discharge under supervision of a Masters‑level clinician.

Complete initial discharge planning and risk assessments under supervision of a Masters‑level clinician.

Integrate assessment information with the attending physician’s initial treatment plan; compile a Master Treatment Plan with individualized, measurable, observable problems, interventions, and short‑term goals; update the plan as patients progress.

Assess and document patient’s ongoing progress toward goals and adjust the discharge/safety plan as appropriate.

Complete a Chemical Dependency Assessment as needed under supervision of a Masters‑level clinician.

Case Management

Engage, collaborate, and problem‑solve with patients, families, treatment team members, and community providers regarding status and discharge progress.

Ensure patient attends appropriate level of care or program as indicated on the treatment plan and facilitate transitions when status changes.

Interface with staff of the appropriate level of care to facilitate smooth transfer.

Maintain necessary documentation of all services and quality of care.

Communicate and enforce documentation requirements to all staff to meet accreditation and certification body regulations.

Maintain knowledge of a variety of resources to assist in patient care and promote effective resource use.

Follow through on case management tasks such as discharge planning, documentation, and chart compliance auditing.

Ensure patient rights are upheld; advocate for the lowest level of care consistent with patient needs; function as a resource for patients, families, and significant others.

Coordinate high‑risk cases requiring reporting and coordination with other agencies; facilitate family meetings as appropriate to enable safe discharge with crisis plans.

Use current systems and materials to provide “hand‑off” communication.

Discharge Planning

Coordinate discharge plans with patient/family/significant others identified by the treatment team.

Provide, monitor, and document discharge activities including identification of needs, patient involvement, referrals for aftercare, and coordination of referrals to other agencies.

Identify and provide appropriate resources for potential risk factors and patient needs.

Organize resources to keep patient as close to home as possible; liaise with aftercare providers and follow up after discharge to ensure plan efficacy.

Treatment Planning

Provide support and assistance to patients and their support systems.

Formulate an initial discharge plan from assessment data.

Assist in episode of care planning to identify services if patient requires increased or decreased intensity.

Communicate with attending practitioner, treatment coordinator, and other providers to ensure continuity of care.

Provide feedback to the treatment team on certification of days/services.

Knowledge of and Adherence to Standards for the Care of Specific Developmental Stages

Adolescent Patients (13‑18)

– Maintain knowledge of program rules, protocols, and handbooks; assess behaviors; recognize acting out behaviors; demonstrate awareness of learning disabilities; maintain knowledge of related systems of care and resources; facilitate family meetings; act as a resource to staff.

Adult Patients (19‑64)

– Gather information, use judgments based on adult behaviors; recognize appropriate limits; maintain knowledge of rules, protocols, and resources; facilitate meetings; utilize theoretical concepts for effective practice.

Geriatric Patients (65+)

– Maintain knowledge of developmental tasks, sociological theories of aging; utilize gerontological theory for interventions; gather appropriate information; maintain knowledge of geriatric resources; facilitate meetings; maintain knowledge of treatments, medications, and side effects.

Chemically Dependent Patients

– Assess needs, document plan, collaborate for discharge and follow‑up, maintain knowledge of resources, awareness of co‑dependency impact, establish limits on medication‑seeking behaviors.

Culturally/Spiritually Diverse Patients

– Recognize multicultural differences, demonstrate awareness of communication styles, show willingness to adapt behavior, and adhere to diversity principles.

Relationship With Others

Function effectively as a team member with tact, sensitivity, sound judgment, and a professional attitude.

Address patients, visitors, physicians, and coworkers pleasantly and respectfully; maintain favorable working relationships.

Respond to patients with empathy and positive interpersonal skills.

Demonstrate flexibility, assume added tasks, be receptive to feedback, and act on it.

Maintain department clean and safe.

Comply with infection control, OSHA regulations, risk management, and employee code of ethics.

Attend regular staff meetings and departmental functions.

Maintain attendance record consistent with hospital attendance policy.
