
Care Manager - Young Adult Program

Overview
Pay:
$20-22 per hour (DOE)
Schedule:
Full-Time, Monday -Friday 8AM – 4PM or 9AM – 5PM
Location:
130 Calo Ln, Lake Ozark, MO 100% On-site
The Care Manager serves as the primary point of contact and advocate for families after a student’s initial enrollment period. This role is central to care coordination, communication, and collaboration across clinical, residential, academic, and administrative teams to ensure a supportive, seamless treatment experience.
Responsibilities
Care Coordination & Family Support:
Act as the primary liaison for parents, school districts, and referents; provide proactive, timely communication and support.
Treatment Team Collaboration:
Coordinate and support Treatment Team Meetings, documentation, and interdisciplinary communication.
Enrollment & Onboarding:
Support new student and family onboarding and ongoing transition processes.
Administrative & Clinical Support:
Provide logistical and administrative support to the Clinical Department and assist with outcomes tracking and documentation.
Relationship Management:
Build and maintain trusting relationships with families, internal teams, and external partners.
Qualifications
Must be at least 21 years of age
Bachelor’s degree in a related field preferred with a strong preference for Bachelors of Social Work (BSW), or 1+ year of experience in a residential treatment or behavioral health setting
Strong communication, organization, and collaboration skills
Ability to pass background check
Experience working with families, youth, or clinical teams strongly preferred
