Job Description:
Enhanced Community Care Management (ECCM) is seeking a dedicated Geriatric Clinical Pharmacist to join our expanding team, focusing on optimizing medication regimens and enhancing quality of life for complex geriatric and palliative care patients. This role emphasizes proactive deprescribing strategies and patient‑aligned goals to reduce polypharmacy and burden, particularly at clinical inflection points. The successful candidate will be an embedded team member, leveraging strong interdisciplinary collaboration skills to actively influence providers and implement clinical interventions that significantly improve patient outcomes and contribute to ECCM's commitment to goal‑concordant care.
ESSENTIAL RESPONSIBILITIES
Review gaps in care for high risk patients as part of a multi‑disciplinary team. Conduct research including gathering claims data, reviewing clinical websites, etc. to thoroughly evaluate each patient's unique situation to provide the appropriate clinical recommendation.
Contact patients, caregivers, physicians, pharmacists, etc. to intervene on identified gaps of care using a high level of clinical judgement and discretion.
Other duties as assigned.
EDUCATION
Required : Bachelor's Degree in Pharmacy
Preferred : Doctorate in Pharmacy
EXPERIENCE
Required : Required experience can be reduced from 3-5 to 1-2 years if the candidate completed a clinical residency program.
5 - 7 years in Pharmacy
3 - 5 years in Pharmacist - Utilization Management
3 - 5 years in Pharmacist - Medical Therapy Management
Preferred :
Completion of a PGY-2 ASHP accredited Pharmacy Residency Program
3-5 years in palliative care
3-5 years in geriatric care
LICENSES AND CERTIFICATIONS
Required : Registered Pharmacist
Preferred : Board Certified Geriatric Pharmacist (BCGP), Age-Friendly Pharmacist
