We currently have an urgent need for a Physical Therapist for a full-time assignment at a skilled nursing facility in Stockton, CA.

Monday-Friday 8am-4:30pm
Job Description
Evaluating patients’ physical abilities and limitations
Developing individualized treatment plans
Providing therapeutic exercises, gait training, and balance activities
Using equipment or modalities to reduce pain and improve function
Monitoring progress and adjusting care plans as needed
Educating patients and caregivers on safe movement and fall prevention
Collaborating with nurses, physicians, and other rehab staff
Perform discharge planning and recommend appropriate post-discharge care or equipment
Train patients in activities of daily living related to mobility (bed mobility, transfers, ambulation)
Identify fall risks and implement preventative strategies
Assist with pain management through manual therapy and therapeutic modalities
Ensure compliance with facility policies, state regulations, and documentation standards
Participate in care plan meetings and provide progress updates to the interdisciplinary team
Supervise and delegate tasks to Physical Therapist Assistants (PTAs) and rehab aides
Maintain accurate and timely documentation for billing and regulatory compliance
