Day shift, 5x8 hours (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.
Job Description
The Home Health Physical Therapist will provide skilled care to patients in their homes, ensuring mobility and functional independence through evaluations and therapeutic interventions. The therapist is expected to transport themselves between patient locations and maintain high productivity standards.
Responsibilities
Deliver physical therapy services according to prescribed treatment plans.
Conduct patient assessments and evaluations.
Develop and implement individualized treatment plans.
Document patient progress and modify treatment plans as necessary.
Educate patients and families on treatment protocols and home exercises.
Maintain up-to-date knowledge of best practices in home health therapy.
Required Experience / Certifications / Licensure
1 year of skilled home health experience required.
Oregon Physical Therapist license required.
BCLS certification required.
Experience with OASIS documentation preferred.
