Perform patient assessments, H&Ps, and care planning
Manage acute and chronic conditions in post-acute settings
Coordinate care with nursing staff and interdisciplinary teams
Educate patients and families
Maintain accurate and timely documentation
Qualifications
MD/DO with Michigan license and DEA (or ability to obtain)
IM, FM, or Geriatrics background preferred
Post-acute, inpatient, ED, or clinic experience welcomed
