$64,000 - $70,000 annually
LISW / LPCC:
$72,000 - $80,000 annually

Position Summary
This position provides direct care services to clients with mental health crisis needs. These services are provided through individual, group and family therapy, through the diagnostic assessment process or through individual community based rehabilitative services and/or group rehabilitative services including linkage and advocacy as necessary. Services are provided in accordance with governing laws, regulations and certification requirements and must be identified on the Individual Treatment Plan (ITP). This position is part-time, 12-hour day shift with call requirements. This includes evening, night, weekend and Holiday rotations.
Key Responsibilities & Role Highlights
Provides prescreening for hospitalization. Responds to community locations including but not limited to hospitals, universities, and in residential settings if needed to provide crisis intervention services.
Obtains status and serves as a health officer in designated counties.
Documents all services provided and maintains records utilizing the electronic record in accordance with agency policy/procedure.
Education & Experience Requirements
Requires a bachelor’s degree in social work or a master’s in counseling. Master’s degree in social work or counseling preferred.
LSW/LPC required.
Must demonstrate knowledge of mental illness, evidence-based practices, motivational and crisis intervention skills.
Must demonstrate knowledge in chronic physical health issues and needs of persons who have SPMI.
Must demonstrate knowledge of care planning and care coordination in integrated health.
Must demonstrate knowledge of evidence based and developmentally appropriate screening.
Must establish and maintain effective working relationships.
