Position:
Clinical Manager – Home Health
Salary:
$92,000 - $100,000 annually, depending on experience
Service Area:
Twin Cities Metro area. Plus mileage reimbursement of 72.5 cents per mile.
Schedule:
1.0 FTE, Full‑Time, Monday – Friday, 8:00am-5:00pm (weekend rotation every 8‑9th weekend, holiday rotation 1‑2 per year, and shared on‑call rotation with other CLMs)

Position Functions & Responsibilities:
Coordinate and oversee all direct and indirect client services provided by clinical organization personnel.
Provide guidance to the clinical team to continually improve all aspects of home health care services.
Ensure compliance with all local, state and federal laws regarding licensure and certification of personnel and maintain compliance with Lifespark Home Care standards.
Manage the clinical team.
Assist in assessment, planning, implementation and evaluation of client and family/caregiver care for all clinical personnel as indicated.
Complete assigned HCHB workflow tasks including episode management.
Oversee the maintenance of client clinical records, statistics, reports and records for evaluation and reporting of organization activities.
Foster strong relationships with clients and referral sources; address and resolve complaints efficiently to ensure a satisfactory outcome.
Maintain clinical records in compliance with local, state and federal laws.
Maintain adequate and appropriate inventory supplies and equipment for the provision of client services.
Interpret operational indicators to detect census changes and adjust staffing, revenues or expenses accordingly.
Evaluate performance of the clinical team.
Develop skills and techniques in evaluating the performance of clinicians.
Hire, evaluate, coach and terminate organization personnel.
Conduct clinical performance evaluations annually or more frequently if indicated.
Plan and implement in‑service and continuing education programs to meet education and training needs of personnel.
Assist with organization QAPI programs, including evaluation of performance via performance‑improvement programs, productivity, quarterly and annual reviews.
Ensure the quality and safe delivery of home health services.
Develop, implement and evaluate the orientation program for new organization personnel; oversee orientation directly or delegate to another staff member.
Assist in the development of organization goals; develop, recommend and administer organization policies and procedures.
Stay informed about changes in nursing, therapies and home health care; share information with appropriate personnel.
Promote home health referrals in the health care community.
Other duties as assigned.
Manager on Duty responsibilities: manage safety concerns for clients and clinicians, escalating when appropriate.
Participate in weekend and holiday rotation as Manager on Duty.
Provide direct patient care when needed.
Qualifications:
Minimum of three years of experience working with people in the second half of life.
Minimum of five years of experience in a leadership capacity for Medicare Home Care (preferred).
Demonstrated ability to supervise and direct professional and administrative personnel.
Experience teaching, training and evaluating clinical staff (preferred).
Experience in customer service / direct client contact field.
Active professional licensure as a Registered Nurse (RN), Physical Therapist (PT) or Occupational Therapist (OT) in the state of Minnesota.
Working knowledge of OASIS and ICD‑10 coding.
