Integrated Care Management Manager- Behavioral Health

Indiana University Health is seeking individuals who embody these values to join our Behavioral Health leadership team in the role of Integrated Care Management Manager.

The

Manager of the Virtual Integrated Behavioral Health Collaborative Care Management (CoCM) Team

is the operational and clinical leader responsible for delivering integrated behavioral health services with fidelity to the CoCM model. The manager oversees

day-to-day program operations , partners with

ambulatory/primary care sites and psychiatric consultants , and ensures consistent workflows that support access, engagement, and outcomes. Key duties include

clinical and administrative supervision , coordinating

weekly case reviews , leading team meetings and training, managing

hiring and performance evaluations , and driving

quality/outcomes reporting

and improvement work. The role also includes

budget oversight , compliance with applicable policies/standards of care, service recovery/complaint resolution, and support for

program expansion and external engagement .

Provides direction and leadership in case management and/or social services to ensure effective care for patients. Evaluates and coordinates the area's services and the needs of the patients and the community served. Promotes effective utilization of hospital resources. Ensures processes support appropriate reimbursement for services rendered. Assists with program direction and policy development, as well as quality improvement initiatives. Collaborates with the multidisciplinary team in order to ensure efficient and purposeful integration of services for the patient. Responsible for safe, efficient and effective care transitions, and ensures the patient and family are engaged in the plan of care. Reports to: Executive Director, Virtual Care

Direct Reports: 15

Key Relationships: Primaryr Care Physicians, Psychiatrists

Candidate Qualifications

Candidates for the position will be evaluated according to the following criteria and competencies.
Requires a Bachelor's Degree in Nursing with a Master's Degree in Nursing preferred, or requires a Master's Degree in Social Work, for all incumbents newly hired or promoted into the position after September 15th, 2025. Incumbents hired prior to this date require a Bachelor's Degree in Nursing with a Master's Degree in Nursing preferred, or require a Bachelor's Degree in Social Work with a Master's Degree in Social Work preferred.
Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license, or requires a Licensed Social Worker, LCSW preferred. If Licensed Social Worker, LCSW must be obtained within 2 years of hire into the position.
Certification in Case Management preferred.
Requires strong clinical assessment; planning; and goal development skills.
Requires knowledge of Case Management principles and standard practices.
Requires knowledge of community resources and appropriate venues for care delivery.
Requires knowledge of state and national regulatory agency guidelines.
5-7 years of relevant experience required.
Requires a strong clinical background with relevant leadership experience supporting patient care transitions.
