
Participant Care Coordinator - TRU - PACE - Clinic

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Pay:
$20.25 to $25.61 per hour
Schedule:
Monday - Friday, 8:00am-4:30pm
Location:
4 days in-office in Lafayette, 1 day from home
JOB SUMMARY
The PACE Clinic Scheduler/Care Coordinator is responsible for scheduling all appointments with specialists and medical clinic appointments. Coordinates and maintains appointment data sheets to make certain that CMS regulations are followed to ensure that all participants are receiving specified care and appointments. Collaborates with outside providers to establish a working relationship to provide appointments and care for participants. Collaborates with PACE Medical Records office to confirm that provider notes are being received in a timely manner. Participates in clinic meetings and overall planning related to the clinic.
EDUCATION AND QUALIFICATIONS
Must have 5 years administrative experience in medical related field
Must be computer proficient and able to work with various databases, and be competent with Word, Excel and PowerPoint programs
Ability to collaborate with team and manage several projects simultaneously
Experience working with the frail or elderly population preferred
Experience working with individuals with mental health needs desirable
ESSENTIAL DUTIES
Assist in prioritizing the scheduling of participants.
Schedule all appointments for participants with specialists and outside medical appointments when requested by the medical team. Will create authorizations for scheduled appointments within the EMR.
Documents in the medical record as appropriate to scope of practice.
Perform any necessary filing or clerical duties necessary to complete charting requirements.
