SBH Health System is seeking a compassionate and organized
Pediatric Collaborative Care Coordinator
to support youth ages 12-21 across our three pediatric ambulatory care sites. This role ensures timely access to behavioral health services, smooth care transitions, and strong collaboration with
Pediatricians, Social Workers, and Behavioral Health providers .
Responsibilities
Coordinate mental health referrals, follow-up, and care transitions across all pediatric sites
Partner closely with Pediatricians and Social Workers to support integrated behavioral health care
Provide youth-centered education on mental health conditions, coping skills, and treatment plans
Assist with mental health screenings and ensure timely follow-up for elevated scores
Support crisis response, safety planning, and linkage to urgent services
Connect families to community mental health, school-based, and social service resources
Maintain accurate documentation and track high-risk youth for follow-up
Participate in interdisciplinary case conferences and quality improvement initiatives
QUALIFICATIONS:
Education Required - HS Diploma or Equivalence
License/Certification/Registration(s) Required - Graduate from an Approved Medical Assistant Program
Experience/Skills Required
Previous experience in a medical setting or 2 years relevant work experience preferred
Excellent communication/ interpersonal skills
Bilingual preferred
Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.
