We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long‑term outcomes—not rushed visits and volume-driven metrics.

In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.

What You’ll Do

Provide comprehensive primary care services including preventive, wellness, and acute care visits

Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines

Develop and oversee longitudinal care plans focused on prevention and chronic disease optimization

Manage medically complex patients and address social determinants of health

Prescribe medications and therapies within scope of practice

Collaborate with nurses, care coordinators, behavioral health providers, and specialists

Use population health data and quality metrics to close care gaps

Educate and empower patients to actively participate in their care

Compensation for this role includes:

Competitive base salary aligned with experience, scope of practice, and panel responsibility

Value-based incentive opportunities tied to quality measures, preventive care, chronic disease management, and patient engagement

Incentives designed to reward thoughtful, proactive care rather than RVU production

For experienced APPs, incentives reflect independent panel management, longitudinal patient outcomes, and contribution to population health goals.

For newer or early-career APPs, the compensation model emphasizes stability, structured onboarding, mentorship, and a gradual ramp-up of panel and incentive expectations.

Our goal is to align compensation with meaningful primary care work—supporting providers who take the time to deliver comprehensive, patient‑centered care.

Requirements
Required Qualifications:

Graduate of an accredited NP or PA program

Current unrestricted APRN or PA license

National board certification

BLS certification

Preferred Qualifications:

Primary care or outpatient experience

Chronic disease management experience

Interest in value-based care and population health
