Job Description

This job functions as the liaison and communicator with the patient, caregivers, healthcare providers, and multidisciplinary team members as well as post‑acute care and third‑party payers. It discusses alternative care options with patients, caregivers, and the team, assists with discharge planning needs, and facilitates movement along the health care continuum to ensure quality, cost‑effective outcomes in collaboration with the multidisciplinary team.

Responsibilities

Manage various aspects of the planning process in anticipation of the patient’s movement through the health care system.

Use care management policies and procedures, including application of evidence‑based criteria and level of care, and ensure continued appropriateness.

Manage the care of assigned patients through the health care system based on the patient’s individual needs.

Prepare, execute, and reinforce post‑discharge care plans.

Adapt communication style to the specific patient population, respecting privacy, and tailoring explanations of services or procedures.

Perform other related duties as required.

Knowledge, Skills and Abilities (KSAs)

Proficiency in using computers, software, and web‑based applications.

Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.

Good organizational and time‑management skills.

Conflict‑resolution skills and ability to demonstrate good judgement and decision making in performing tasks.

Education

Graduate of an accredited school of nursing.

Preferred: Bachelor’s degree in nursing.

Work Experience

Required: 2 years of related experience.

Certifications

Required: Current registered nurse (RN) license in state of practice.

Basic Life Support (BLS) through the American Heart Association.

Preferred: Certification in Case Management (CCM).

Health Care Management (CPHM).
