At Select Home Health & Hospice, we believe in putting patients first: *“Select the best for your health!”***About the Role**We are seeking a compassionate and reliable Hospice Certified Nursing Assistant (CNA) to join Select Hospice. In this role, you will provide hands-on personal care to patients in their homes, focusing on comfort, dignity, and quality of life. You will support patients and their families during end-of-life care while working under the direction of a Registered Nurse (RN).**Responsibilities*** Provide personal care including bathing, dressing, oral care, feeding, and elimination* Assist with ambulation, positioning, transfers, and ADLs* Measure and document vital signs and observations as directed* Travel to and care for **multiple hospice patients per day** in their residences, following individualized plans of care* Observe and report changes in patient condition or home safety concerns* Follow RN care plans and agency policies* Maintain patient confidentiality and HIPAA compliance* Complete required trainings, meetings, and in-services* Support patient comfort through socialization and emotional presence**Physical Requirements*** Ability to sit, stand, and drive for extended periods, with or without reasonable accommodation.* You must be able to lift to 15 pounds occasionally, with or without reasonable accommodation.* Regular use of phone, computer, and presentation tools.**Disclaimer:***This description reflects the general nature and level of work for the Account Executive position.
