About the Role
As a Home Health CNA with Pathfinder, you’ll provide personal care and daily living assistance under the guidance of a Registered Nurse (RN) and in accordance with each patient’s individualized care plan. You’ll work one-on-one with patients in their homes—helping them maintain independence, dignity, and comfort—while being supported by a collaborative, responsive clinical team.
Essential Job Functions
Provide hands‑on personal care, including bathing, grooming, toileting, dressing, and feeding, in accordance with the individualized Hospice Aide Plan of Care
Maintain patient comfort and dignity while supporting physical and emotional needs during end‑of‑life care
Accurately document all care and services provided, ensuring compliance with agency policies and hospice standards
Communicate changes in patient condition or care needs promptly to the nurse case manager or designated supervisor
Operate and assist patients with the use of durable medical equipment (e.g., walkers, wheelchairs, oxygen) safely and appropriately
Monitor and report any safety concerns or unusual observations regarding the patient’s condition or environment
Collaborate closely with the interdisciplinary hospice team to ensure continuity and quality of care
Provide companionship and a calming presence to patients and families, reinforcing a sense of comfort and trust

Requirements
Must be 18+ years old
Active Texas CNA license
Current CPR certification
Valid driver’s license, reliable vehicle, and auto insurance
Able to lift up to 50 lbs
Clear background check
Home Health or Hospice experience required
Strong communication and independent work skills
