Description
The Home Health Registered Nurse (RN) provides high-quality, patient-centered care in a home setting. This role applies clinical expertise and nursing principles to assess, plan, and deliver care while ensuring coordination with a multidisciplinary healthcare team. The RN plays a key role in supporting an integrated home health system across Wayne, Oakland, and Macomb counties.
What You’ll Be Doing
Deliver direct nursing care to patients in their homes, ensuring safety, comfort, and quality outcomes
Conduct comprehensive patient assessments and develop individualized care plans
Evaluate the need for additional services such as physical therapy, occupational therapy, speech therapy, nutrition, and social work
Coordinate care with interdisciplinary team members to ensure a seamless patient experience
Manage an average caseload of approximately 6 patient visits per day
Travel within designated service areas (40–60 miles daily)
Maintain accurate and timely clinical documentation
Educate patients and families on care plans, treatments, and health management
Perform other related duties as assigned
What You Must Have
Current Registered Nurse (RN) license in Michigan
Active BLS certification through the American Heart Association (AHA)
Minimum of 1 year of Home Health RN experience
Valid driver’s license
Reliable transportation and willingness to travel locally
