How You’ll Make an Impact
The RN Care Manager works closely with complex renal patients in their home, dialysis center, by phone, and electronically. Primary focus is to improve patient outcomes by securing permanent access, promoting home dialysis modalities & kidney transplantation, educating patients on self‑management, addressing comorbid risks, and coordinating care.

Responsibilities:

Conduct comprehensive assessments of medical, behavioral, pharmaceutical, and social needs; perform ~12 assessments per week and manage ~150 patients.

Create and implement individualized care plans with patient, nephrologist, PCP, and team.

Act as advocate and resource for patient, family, and provider(s).

Facilitate care across settings: home, hospital, skilled nursing, and acute care facilities.

Manage transitions of care to reduce avoidable readmissions.

Assess patient knowledge of renal condition and provide education and self‑management support.

Provide ongoing reassessment and follow‑up to improve outcomes.

Offer clinical oversight to non‑licensed support teams (community health workers, health coaches) and licensed teams (social workers, renal dietitians), delegating tasks as appropriate.

Qualifications

1+ years of nursing experience in case or care management, preferably coordinating across multiple settings.

2+ years of healthcare related experience.

Current, unrestricted compact Registered Nurse license.

Valid BLS certification from an AHA or American Red Cross training facility.

Commitment to patient‑centered care and proactive patient advocacy.

Ability to empower patients in self‑management and shared decision making.

Strong written and verbal communication with cultural awareness.

Analytical, critical thinking, community engagement, and facilitation skills.

Travel flexibility throughout the assigned region and willingness to conduct home visits as needed.

Preferred Qualifications

Bachelor’s degree in nursing.

Empathy, enthusiasm, sense of humor, and strong work ethic.

Experience with vulnerable patient populations (ESRD, geriatrics, minorities, low‑income, uninsured).

Rapport building through inquiry and listening.

Familiarity with electronic medical records.

Community outreach experience.

Competence with MS Office and telecom devices.
