Care Navigator

DEFINITION:

As a part of the outpatient treatment team model, Care Navigators will report directly to an Integrated Team Manager. The Care Navigator will be responsible for ensuring that the client's agreed upon services are completed at the frequency that they are outlined in the client's person-centered treatment plan. The Care Navigator must exhibit both organizational and professional communication skills to communicate effectively, both with internal and external providers. Care Navigators will also be expected to regularly generate reports and communicate needs to applicable treatment team members. A high degree of attention to detail is required.

EDUCATION AND EXPERIENCE:

• Case Management II certification; or

• High school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance abuse issues; or

• 60 college credit hours in any field and 12 months of behavioral health experience; or

• Bachelor's or Master's degree in any field with 6 months of Behavioral Health experience; or

• Bachelor's or Master's Degree in a Behavioral Health field

KNOWLEDGE AND SKILLS:

• Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.

• Ability to establish working relationships with clinical, administrative and support staff in order to develop professionalism and coordination within the team.

• Ability to establish and maintain an effective working relationship with fellow employees, clients and community partners.

• Ability to interact with staff, clients and administration and the community both in person and through electronic communication.

• Ability to maintain strict confidentiality guidelines regarding client information.

• Ability to work well under pressure meeting deadlines with efficiency and consistency.

• Ability to work with various systems while maintaining professionalism, maturity, objectivity and a cooperative attitude.

JOB DUTIES AND RESPONSIBILITIES

:

This position is responsible and held accountable for the following duties:

• Ensure all care coordination activities meet ODMHSAS and CCBHC standards.

• Facilitate coordination with primary care, specialty providers, hospitals, schools, child welfare, and other community services.

• Document Letters of Collaboration and maintain updated records of external provider involvement.

• Working knowledge and relationship with available area resources for quick and impactful collaboration.

• Understands and promotes our Certified Community Behavioral Health Clinic (CCBHC) concept by providing or supporting community-based mental and substance use disorder services; advancing integration of behavioral health with physical health care; assimilating and utilizing evidence-based practices on a more consistent basis; and promoting improved access to high quality care.

• This job does require the selected candidate to possess a current and valid driver's license, to have a clean driving record and reliable transportation
Regular and predictable attendance is an essential job requirement.
Employees must be willing and able to perform all job-related travel normally associated with this position (some travel or extended hours may be required to support executive or board functions).
Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
Must meet "satisfactory" expectations at any scheduled job performance evaluation.
