POSITION SUMMARY

The Specialty Patient Care Coordinator - Infectious Disease (SPCC-ID) is responsible for ensuring patients-particularly those with HIV/AIDS and other infectious diseases-receive comprehensive, coordinated, and patient-centered care throughout their healthcare journey.

This role combines traditional patient care coordination with specialized case management services, including Ryan White Program compliance. The SPCC-ID works closely with patients, families, healthcare providers, and community partners to facilitate communication, manage individualized care plans, and connect patients to medical, behavioral health, and community resources.

Essential Functions Statement(s)
Patient Support and Coordination
Develop and manage personalized care plans for patients, addressing their unique needs and preferences.
Serve as the primary point of contact for patients and their families, providing guidance and support throughout their care journey.
Facilitate communication between patients, healthcare providers, and other stakeholders to ensure timely and accurate information sharing.
Care Plan Management
Monitor patient progress and adherence to care plans, making adjustments as necessary to ensure optimal outcomes.
Coordinate referrals to specialist, behavioral health services, and community resources.
Assists patients with scheduling appointments, transportation, and other logistical needs.
Identify and address care coordination errors and gaps, working to continuously improve processes and patient outcomes.
Resource Management
Connect patients with appropriate insurance, financial assistance programs, and community resources.
Provide educational materials and resources to patients and their families, empowering them to make informed decisions about their care.
Other duties as assigned.
SKILLS & ABILITIES
Education : Bachelor's degree in social work or related field required.
Experience : 2-4 years of related experience preferred. Experience working with HIV/AIDS or infectious disease populations strongly preferred.
